Medical Forms

All 5th - 8th grade students that plan to play an SFA sport during the  2024 - 2025 school year will need to have a physical and medical form on file with the athletic director.

ALL Athletes (4K - 8th grade) are required to complete both the concussion information sheet and the Sudden Cardiac Arrest information sheet. Forms must be signed and on file annually by all athletes

 

All forms should be dated after 4/15/24 (for the 24-25 School year) and on file prior to the first practice.
