Registration Packet 2020-2021 School Year

 
ONLINE REGISTRATION PACKET 2020-2021

Due to Covid-19 SFA will hold all registration activities online.
 
 
  • All families must be in good standing with the business office in order to register.  Any student with a past due balance in their FACTS account will NOT be included on a class list or receive a class schedule.  Please contacts Mrs. Tina Sosnowski in the Business Office if you have any questions or concerns at tina.sosnowski@sfawolves.org or 901-381-2566

To begin the registration process, review each section below and complete where applicable for your student(s).
 
 
1) MEDICAL FORMS
 
Please complete and return by email to renata.guimaraes@sfawolves.org by AUGUST 1st.  
 
      •  IMMUNIZATION RECORD – from your child’s pediatrician – This form is REQUIRED for ALL Kindergarten, 7th Graders, and ALL Students NEW to SFA (regardless of grade). A student may NOT start school without their immunization record updated.
      •  SFA DISTRIBUTION OF MEDICINE FORM - Distribution of Medicine for any medication (prescriptions or over-the-counter) with a note from the doctor that will include the name of the student, name of medicine, how much to give, time to be given, side effects if any and when to stop giving it. Parents need to sign it. Medication has to have a child name on the original bottle.
      •  ASTHMA ACTION PLAN FORM - If a student has asthma, they need to have an Asthma Plan.
      •  EMERGENCY CARE PLAN - If a student has any Allergies, they need an Emergency Care Plan.
      •  SEIZURE ACTION PLAN - If a student has Seizures, they need a Seizure Plan.

2) SFA CAFE 
 
 
PLEASE NOTE for the 20-21 School Year: 
 
Students in 3K-3rd grades are required to bring their lunch daily. Lunch will not be available for purchase for these students. Students will be eating in their classroom. Microwaves are not available.
 
Students in 4th-8th grades will have the option to bring their lunch daily or purchase a prepackaged lunch from our SFA Cafe. PREPACKAGED LUNCHES MUST BE PREPAID ONE MONTH IN ADVANCE THROUGH YOUR MY PAYMENTS PLUS ACCOUNT OR BY SENDING A CHECK TO THE SCHOOL OFFICE. 
  
      • Complete the form below if you wish to have lunch provided for your student for the Month of August.   
      • Orders and payment are due by Monday, August 10th.   The Full monthly payment of $84 for August must be in your My Payments Plus account or sent to the school office by August 10th in order to receive lunch for the month of August.  The August menu is available below. 
 
 
 
Students in the 4th-8th grades will be eating by grade level in the SFA Cafe or MPR. Microwaves are not available.
 
Parents, Be sure to activate your "My Payments Plus" account if you do not already have one.  Balances will carry over from last year.   See below for step-by-step details.  Student account numbers will be emailed to parents of 4th-8th grade students.  Contact Ms. Cole for more information.

3) TECHNOLOGY REQUIREMENTS
 
ALL PARENTS: Complete the Acknowledgment below to show you have read the SFA Technology Policy and the Parent Responsibility Policies (listed below). Your child will not be issued a device until we have this digital record confirming your understanding of both policies.
 
If you have more than one child at SFA, please fill out a separate acknowledgment for each of your children.
This form serves as your digital signature.
 
4) SFA CHURCH - FIRST COMMUNION & CONFIRMATION REGISTRATION:
 
If your child is in the 2nd or 8th grade and wishes to receive the sacraments of First Communion or Confirmation at St. Francis Church, you must complete this registration form. Sacrament Registration Form - Click here

You may also use this form if your child is in another grade and did not receive their sacraments at the traditional time.

5) PRE-PURCHASED STUDENT SUPPLY BOXES
 
Pre-Purchased Knowledge Tree school supply kits will be delivered to your student's classroom.    If you did not purchase the pre-packaged kit, student supply lists are available on the SFA website by clicking here.
 
6) REQUIRED CHAIR BAGS & HAPPI NAPPI'S FOR PK-5TH GRADE
 
In addition to the school supplies, some students are required to bring the additional supplies requested below for their grade level on the first day of school.  For your convenience, items can be purchased by completing the form below and will be delivered to your child's classroom.  
 
You may re-use an item from a sibling or previous year, if applicable. 
 
 
IMPORTANT:  These additional supplies are not included in the school supply lists or pre-packaged supply kit.
 
7) EXTENDED CARE REGISTRATION

8) SFA ATHLETICS 
 
All 5th - 8th-grade students that plan to play an SFA sport during the 2020–2021 school year will need to have a physical and the medical forms on file with the Athletic Director, Mrs. Kareen Baird along with the Concussion Information Sheet and the Sudden Cardiac Arrest Information Sheet.   Please click the link for the 5th-8th grade Sports Packet:
 
All 4K-4th grade athletes are required to complete both the Concussion Information Sheet and the Sudden Cardiac Arrest Information Sheet. These forms must be signed by the athlete and the parent and need to be on file with the Athletic Director, Mrs. Kareen Baird prior to the start of practice or games.  See the Medical Forms Packet below for students in 4K-4th grades.
 
 
9) SFA Parent Teacher Organization (PTO)
 
Dear Parents,
 
Welcome back! We are so excited that our SFA Family is back together again! As with everything, PTO is going to look different this year. As always, we will do everything we can to support our amazing faculty and staff. We will have quarterly meetings to keep you informed, but they will be virtual for the foreseeable future. If you have any questions, please do not hesitate to email us. We are looking forward to a wonderful year!
