The following forms should be completed for your child and signed by their pediatrician if they will receive medication (prescription and/or over-the-counter) during the school day.  If your child has a medical condition such as asthma, food allergies, or seizures, please complete the appropriate form and return it to the school nurse along with the medication to be dispensed.
SFA WILL BE USING THE DEFINITION OF CLOSE CONTACT FROM THE SHELBY COUNTY HEALTH DEPARTMENT COVID-19 RESPONSE UNIT:
 
Identified Close Contacts: with universal masking applied effective 8/6/2021, close contacts are those students within 3 feet of distance, cumulative 15 minutes, vaccinated and unvaccinated. In cafeterias, close contacts will be identified as those within 6 feet of distance due to inability to wear masks during a meal.
