(Part-time Program students will hold registration during their parent orientation evening on August 7th)
3:00 PM - 7:00 PM IN THE SFA SCHOOL GYM
(You are welcome to register at any time between these hours of 3:00pm and 6:30pm.
The 2018-2019 Registration Packet is now available. Please be sure to review ALL INFORMATION contained on this page and have necessary forms COMPLETED on Registration Day or ONLINE prior to registration day (where applicable). This will ensure a quicker and more efficient registration process!
Registration will be held in the School GYM. Check or FACTS billing will be acceptable payment methods. SEPARATE CHECKS MAY BE REQUIRED.
All families must be in good standing with the business office to Register.
Please review steps below prior to Registration Day and have any applicable forms completed either on-line or ready to turn in.
THE FOLLOWING STATIONS WILL BE SET UP IN THE SCHOOL GYM (Not necessarily in this order):
1) SFA PTO
"Join the Pack"! All SFA families are members of SFA PTO! Your FACTS account will be charged the membership fee in August! Visit the PTO station for Volunteer information and other PTO news. Our parent Volunteers are one of the things that make SFA great!

2) SFA CHURCH- FIRST COMMUNION & CONFIRMATION REGISTRATION:
If your child is in the 2nd or 8th grade
and wishes to receive the sacraments of First Communion or Confirmation at St. Francis Church, you must complete this registration form. Sacrament Registration Form - Click here
You may also use this form if your child is in another grade and did not receive their sacraments at the traditional time.

3) SFA CAFE
COMPLETE THE SFA CAFE REGISTRATION FORM BELOW AND RETURN TO SFA CAFE TABLE AT REGISTRATION.
GRADES 1-8 Parents - Be sure to activate your "mypaymentsplus" account! See Details below! Student account number will be available at registration.
Please note, Cash will no longer be accepted in the SFA Cafe. For your convenience, the SFA is using the "mypaymentplus" program for adding funds to your child's lunch account and tracking purchases. Checks will also still be accepted.
3K, 4K, 5K - August Lunch Menu and Order Form available below - orders due Aug. 8th (can be turned in at registration)

4) STUDENT SUPPLIES/SPIRIT SHOP
-Pre- Purchased Knowledge Tree kits MUST be picked up in the school gym during registration. If you did not purchase the prepackaged kit, lists are available on the school website by clicking here.

b) BE SURE TO STOP BY THE SFA SPIRT SHOP FOR ALL YOUR SPIRIT NEEDS! New Spirit Items will be available for purchase during registration in the School Gym.

6) AFTER SCHOOL ACTIVITES - Fall after school activities are offered for an additional fee. You Do NOT have to be registered in Extended Care to participate in these activities. More information will be available at registration.

7) Medical Forms and other Miscellaneous Forms - Turn in at Registration Table or before to the front office.
a) Immunization Record – from your child’s pediatrician – This form is Required for ALL 3K, Kindergarten, 7th Graders, and All Students NEW to SFA (regardless of grade). Turn in at Registration or before to the front office.

a) ALL GRADES: Read, sign and return the SFA Technology Acceptable Use Policy in order for students to use any SFA electronic device.
b) PARENTS OF 6th-8th Grade Students - in addition to the Form above - must complete the Parent Responsibilities Form below as well as attend a REQUIRED technology parent meeting in order for students to be assigned a laptop on the first day of school.
Schedule of Meetings as follows (located in Middle School STEM Classroom JN111):
3:00 pm, 3:30 pm, 4:00 pm, 4:30 pm, 5:00 pm, or 5:30 pm
Meetings are first come, first serve – will last about 20 min, max attendance is approx 50 adults per session.
**At least one middle school parent per student must attend ONE session. Meetings are for MIDDLE SCHOOL PARENTS only**
Students will not be issued a laptop unless a parent attends a session.
c) SCHOOLOGY Learning Management System Information - visit the Technology Station at registration for parent access codes (for all New families or any current families who have never activated your account).
9) Homeroom class lists will be posted on the gym wall and Class Schedules for 6th-8th graders can be picked up at the Schedules station. Check out the Class Trips on display at this station also!
10) SFA Athletics
All 5th - 8th-grade students that plan to play an SFA sport during the 2018–2019 school year will need to have a physical and the medical forms on file with the Athletic Director. For Fall sports, the forms must be on file by July 23rd. All others can be turned in at Registration if not already completed.
ALL Athletes are required to complete both the concussion information sheet and the Sudden Cardiac Arrest information sheet. Must be signed and on file yearly by all athletes. Medical Form Package for 4K-4th Grades
