We will be holding Registration both ONLINE and in person this year!
- All families must be in good standing with the business office in order to register. Any student with a past due balance in their FACTS account will NOT be included on a class list or receive a class schedule. To begin the registration process, review each section below and complete where applicable for your student(s).
1) MEDICAL FORMS
Please complete and return by email to [email protected] by AUGUST 9th or contact Nurse Debra for arrangements. If you do NOT need medication forms, you do not have to fill them out.

- IMMUNIZATION RECORD – from your child’s pediatrician – This form is REQUIRED for ALL Kindergarten, 7th Graders, and ALL Students NEW to SFA (regardless of grade). A student may NOT start school without their immunization record updated.

- SFA DISTRIBUTION OF MEDICINE FORM - Distribution of Medicine for any medication (prescriptions or over-the-counter) with a note from the doctor that will include the name of the student, name of medicine, how much to give, time to be given, side effects if any and when to stop giving it. Parents need to sign it. Medication has to have a child name on the original bottle.

- ASTHMA ACTION PLAN FORM - If a student has asthma, they need to have an Asthma Plan.

- EMERGENCY CARE PLAN - If a student has any Allergies, they need an Emergency Care Plan.

2) SFA CAFE
PLEASE NOTE for the 2022-2023 School Year:
Students in 2 year old program - Kindergarten will:
- Bring their lunch daily
- Eat in the Multipurpose Room
- Be UNABLE to use Microwaves
- Not purchase hot lunches this year - not even Pizza Friday.

Students in 1st - 8th grades will have the option to bring their lunch daily or purchase lunch from our SFA Cafe with a daily declining balance account through My Payments Plus. To set up a My Payments Account, you will need your child's Student ID - Those will be sent to you by August 6th.
If you already have a My Payments Plus account - please be sure your student has money on the account before the first day of school. If you do not want to use the APP - you can always bring in a check to the front office to have money placed on your student(s) lunch account.
All Students 1st - 8th Grade will:
- Eat in the Cafeteria
- Purchase additional items (Price List and Items will be coming soon)
- 5th - 8th Grade Only - can use the microwave when bringing their own lunch
Parents, be sure to activate your "My Payments Plus" account if you do not already have one. Balances will carry over from last year. See below for step-by-step details. Contact Ms. Cole for more information.

3) TECHNOLOGY REQUIREMENTS - ALL PARENTS
PARENTS - 5TH GRADE AND ALL NEW STUDENTS WILL MEET AUGUST 8TH AT 3PM OR 4PM WITH MRS. TORRES FOR LAPTOP MEETING
ALL PARENTS: Complete the acknowledgment (located at the end of the 2022- 2023 Technology Acceptable Use Policy) below to show you have read the SFA Technology Policy and the Parent Responsibility Policies (listed below). Your child will not be issued a device until we have this digital record confirming your understanding of both policies.
If you have more than one child at SFA, please fill out a separate acknowledgment for each of your children.
This form (located at the end of the 2022- 2023 Technology Acceptable Use Policy) will serve as your digital signature.

4) SFA CHURCH - FIRST COMMUNION & CONFIRMATION REGISTRATION:
If your child is in the 2nd or 8th grade and wishes to receive the sacraments of First Communion or Confirmation at St. Francis Church, you must complete this
registration form.

5) PRE-PURCHASED STUDENT SUPPLY BOXES
Pre-Purchased Knowledge Tree school supply kits will be delivered to your student's classroom. If you did not purchase the pre-packaged kit, student supply lists are available by clicking here.
6) REQUIRED CHAIR BAGS & HAPPI NAPPI'S FOR PK-4TH GRADE
In addition to the school supplies, some students are required to bring the additional supplies requested below for their grade level on the first day of school. For your convenience, items can be purchased by completing the form below and will be delivered to your child's classroom.
Required Happi-Nappi - Grades: PK, JK, & 5K
Required Chair Bags - Grades: JK, PK, 5K, 1st, 2nd, 3rd, and 4th
Required BackPack for Laptops: 5th, 6th, 7th & 8th
We will have the Happi Nappi's and Over the Chair Bags for sale at our in-person registration on August 8th along with someone who will offer embroidery services. We do not however, have supplies to purchase the backpacks. Those will have to be purchased elsewhere. You may reuse an item from a sibling or previous year, if applicable.
7) EXTENDED CARE REGISTRATION

8) SFA ATHLETICS
All 5th - 8th grade students that plan to play an SFA sport during the 2022 - 2023 school year will need to have a physical and medical form on file with the athletic director.
ALL Athletes (4K - 8th grade) are required to complete both the concussion information sheet and the Sudden Cardiac Arrest information sheet. Forms must be signed and on file annually by all athletes

All forms should be dated after 4/15/22 (for the 22-23 School year) and on file prior to the first practice.

9) ART ADVENTURE
If your student(s) are in the 1st through 5th Grades you can sign them up for ART ADVENTURES. This is an AFTER SCHOOL program will begin on Wednesday, September 22nd. See FLYER for more details.
NEED TO KNOW:
- Classes will be held each Wednesday beginning September 21nd - 3:15pm - 4:45pm
- LIMITED SPACE - DEPOSIT of $75 (Both September & October's Program Costs) to hold spot along with filled out form below (Bring to Registration or Turn in to Front Office)

10) Bartlett Dance Factory
Right after school here at SFA. Beginning with Grades 3K and up, you can sign up your dancer for classes (please see the below flyer for days and times).
NEED TO KNOW:
- Each date will be after school beginning the week of August 29th.
- Registration Fee = $20.00 (Checks to be made out to Bartlett Dance Factory)
- $60.00 Monthly Tuition Fee - (Billed Through FACTS)
