Registration Packet 2017-2018

THURSDAY,  AUGUST 3RD for FULL-TIME 3K-8th Grade Students
(You are welcome to register at any time between these hours-remember lines may be longest at 3:00pm when doors open so plan accordingly.)

The 2017-2018 Registration Packet is now available. Please be sure to review ALL INFORMATION contained on this page and have necessary forms COMPLETED on Registration Day! This will ensure a quicker and more efficient registration process!

Registration will be held in the School GYM.   Check, Credit Card or FACTS billing will be acceptable payment methods FOR MOST STATIONS.   SEPARATE CHECKS MAY BE REQUIRED FOR MMI (MEAL SERVICE)  AND SUPPLY PURCHASES.
All families must be in good standing with the business office to Register.   

Please review steps below prior to Registration Day and have any applicable forms completed either on-line or ready to turn in. 

"Join the Pack"! All families are members of SFA PTO this year!   Visit the PTO station for Volunteer information and other PTO news. 
SFA Student & Business Directory - A directory will be printed for all families courtesy of SFA PTO!  It will be printed and sent home with students in early September.  Email for address changes by August 10th.

***Back to School Skate Party - August 6th - Cordova Skate Center - 6:30pm-8:30pm
***Principal Welcome Back Coffee Social- August 8th - 8:00am-9:15am - Drop off your students and head to the multipurpose room to greet Mrs. Scherer, new families, and old friends!
***SAVE THE DATE! SFA's 3rd Annual Best Bites of Memphis - October 14th - 7:00pm - 10:00pm - Look for more information at the PTO table.

**NEW THIS YEAR** Register all of your children and pay on ONE form! If your child is in the 2nd or 8th grade and receiving the sacraments of First Communion, Reconciliation or Confirmation, please follow this link to register online. Sacrament Registration Form - Click here

Full-time 3K, 4K and Kindergarten hot lunch will begin September 1st. Lunch menu and order form will be available at Registration. 

4)  STUDENT SUPPLIES -Pre- Purchased Knowledge Tree kits MUST be picked up in the school gym during registration.   If you did not purchase the prepackaged kit, lists are available on the school website by clicking here.
   Note: Students PK-5th may drop their supplies off in the classroom on Registration Day.

Note: All 6th -8th grade students will be given a new Laptop Backpack this year! No other supplies needed.

6)  AFTER SCHOOL ACTIVITES - Fall after school activities are offered for an additional fee. You Do NOT have to be registered in Extended Care to participate in these activities.   More information will be available at registration.

7) Medical Forms -  Turn in at Registration or before to the front office. 
a) Immunization Record – from your child’s pediatrician – This form is Required for ALL 3K, Kindergarten, 7th Graders, and All Students NEW to SFA (regardless of grade). Turn in at Registration or before to the front office.
b) SFA DISTRIBUTION OF MEDICINE FORM complete form below,  if applicable and turn in at Registration
LAPTOPS FOR RETURNING SFA STUDENTS in 6th,  7th & 8th Grade - Returning Students will pick up their school issued laptop AT REGISTRATION  between the hours of 3pm and 7pm.   BE SURE TO CAREFULLY READ AND RETURN THE TECHNOLOGY AGREEMENTS BELOW.
NEW STUDENTS TO SFA in 6th, 7th and 8th Grade - Must stop by the Technology Station for a quick orientation and to receive your laptop anytime during registration hours. 
ALL GRADES:  Read, sign and return the SFA Technology Acceptable Use Policy in order for students to use any SFA electronic device.
SCHOOLOGY Learning Management System Information - visit the Technology Station at registration for parent access codes (for all New families or any current families who have never activated your account).
9)  Homeroom class lists will be posted on the gym wall and 
Class Schedules for 6th-8th graders can be picked up at the Schedules station.  Check out the Class Trips on display at this station also!
10) Don't Miss the SFA SPIRIT SHOP - New items in Time for the New Year.   A Spirit Shop station will be set up in the Gym. 
11)  New this year: VIsit the "SFA Selfie" station for fun pictures! 
12) Be Smart, Drive Safe - Required for all field trip drivers-see form below
13) SFA Wolf Bucks Order Form - Earn Tuition credit - order forms can be turned in at Registration-see form below
14) SFA Athletics 
All 5th - 8th grade students that plan to play an SFA sport during the 2017–2018 school year will need to have physical and medical forms on file with the athletic director. These can be turned in at Registration if not already completed. Medical Form Package for 5th-8th grades
ALL Athletes are required to complete both the concussion information sheet and the Sudden Cardiac Arrest information sheet. Must be signed and on file yearly by all athletes. Medical Form Package for 4K-4th Grades
